Generally, any confinement, which requires intensive care and a specialized nursing care support, may be referred to APNCC. Examples of confinement that normally require special nursing care are stroke, heart bypass, kidney transplant, dialysis, chronic obstructive pulmonary disease, cancer, and conditions when patient is expected to experience extreme pain.
What is Hospice Care? Hospice Care is a special course of treatment for patients who become terminally ill. While cure remains uncertain, the nursing care concentrates in alleviating the physical pain and anxiety of the patient and the family. Without abandoning hope for cure, hospice care concentrates in improving the quality of the remaining life of the patient with his family. The composite care providers take very active role in hospice care. Anxiety alleviation takes a totally new dimension in nursing care by preparing the patient for his ultimate journey. When are patients referred to APNCC? Cases may be referred to APNCC upon the start of a patient’s hospitalization. Nurses engaged while the patient is still in the hospital are contracted as Private Duty Nurses (PDN) under the APNCC. The attending physician will recommend a PDN depending upon his assessment of the patient’s condition. However, the family may contract directly if they so desire. Must nursing care be continued even after discharge from the hospital? As a general rule those who suffer from acute illness must continue to have post-acute nursing care until such time that the patient can care for himself. The need for nursing care is also emphasized when the continuing care will entail the use of medical equipment such as ventilators, cardiac monitors, dialysis, blood glucose monitor, and the like. Should the post hospitalization care be in the home or in APNCC facilities? APNCC provides post-hospitalization care either in the patient’s home or by admitting the patient as resident in the APNCC nursing home. APNCC home care will entail the engagement of 24 hours nursing service of two shifts. APNCC will provide for either rental or outright sale of the required support medical equipment. APNCC nursing home can accept patients for residency care. Families are allowed to visit and share time with their patient. APNCC is complete with emergency and support medical devices. A resident physician is maintained on a 24 hour basis. What are the terms of engagement of APNCC? Engagement for post-hospitalization care shall be in accordance with the endorsement of the attending physician. In the absence of such endorsement, APNCC will prescribe the minimum days of engagement depending on its assessment of the patient’s conditions. |
